On-Line Membership Application

 

      Being desirous of election to membership in your organization,  I / We hereby submit this on-line application, and at my earliest, opportunity, I shall submit a paper application, by mail. I understand, that, if elected, promise to abide by the Rules and Regulations as formatted by the Board of Directors.

I / We understand that written resignations can only be accepted at the Annual Meeting of the Corporation, which is held during the April after each Carnival, and then only if the membership dues have been paid in full.

It is understood that if accepted, I / We shall immediately become obligated to the Eagles Nest, Inc., for the Annual dues of $320.00 per person, which must be paid in the following manner:

Initial payment of Seventy - Five dollars ($75.00), (Which becomes non-refundable once accepted into the membership) with application and balance in equal monthly installments, but no later than October 1. This arrangement unless acted upon by the board of directors, shall continue, in the same manner each succeeding year.
